Call 911, or go to the nearest emergency room. Not us, and not the emergency room you usually use — the nearest one. Minutes matter more than continuity.
If you are having thoughts of harming yourself, call or text 988 — the Suicide and Crisis Lifeline, any hour.
For a poisoning or a suspected overdose: 1-800-222-1222, Poison Control.
A fever in a baby under one month is an emergency at any hour, and that is the one threshold on this site where a delay of hours matters.
Our page on where to go covers everything in between, and it is the most useful page here for a family that is not sure.
Call us. It is the fastest route, and someone can tell you what is available today rather than in two days.
What to have ready: who the appointment is for, roughly what it is about, and whether you have been here before. If it is about something that has been going on a while, say so — that changes how long we book.
If you need to be seen today, say that when you call rather than taking the first slot offered. It is a different conversation and it is a reasonable one to have.
Existing clinic patients can send an appointment request at any hour and our staff will confirm a time with you. Best for routine visits rather than same-day needs.
A request is not a confirmed appointment until we reply. If you need care today, call instead.
Not a rule, just what works better another way:
1720 East Boston Street, Gilbert, AZ 85295. We see families from Gilbert, Chandler, Mesa and Queen Creek.
→ Map and directions ↗For appointments, records, refills, billing and general questions. Do not use this form for anything urgent, and do not put medical details in it — the notice below says the same thing and it is there for a reason.
Please do not include medical details, symptoms, insurance ID numbers, or any other personal health information in this form. For medical concerns, call the office. In an emergency, call 911.
If you were seen somewhere else — an emergency room, an urgent care, a specialist — tell us afterwards. We can only join up your care if we know it happened, and a visit we never hear about is a gap in the record that matters later.
If something went wrong here, tell us. Ask for whoever handles it. That is more useful to us than a review, and it is the only way anything changes.
Prototype note: the form is visual only — submission wiring happens in the WordPress build.
